Although we always prefer to see our patients in person for a consultation before a surgical procedure, we understand that it isn’t always possible or practical. Maybe your schedule is simply too packed, or you live too far away. The consultation fee to meet with Dr. Chouake is $250. This is collected at the time of scheduling and will be applied towards your surgical procedure. There is a $100 booking fee to schedule with any of our non surgical PAs and RN’s. This booking fee is applied towards any service or procedure with us. Due to the nature of of running a very busy practice, we require 48 hour notice to cancel or reschedule or the above fees will be forfeited.
